In this book, the author describes these transitions and re-defines Transitional Care, providing us with a more complex and holistic understanding of the concept, capturing both the physical and existential experience of patients and, importantly, the perspectives of health professionals who support patients at each level of care and after discharge. This renewed understanding provides us with an opportunity to more effectively implement holistic care, taking into account patients’ perspectives and experiences. It ensures that both the physical and the mental health consequences of a critical care event are understood and that our interventions are helpful and therapeutic.
